Start your White Card - Card Re-issue Request

This request should be completed by a student wanting to obtain a reissue of their White Card. This only applies for White Cards issued by QLD and WA.
To complete this form you must one of the below criteria:
1. The original card was issued by Allens Training Pty Ltd RTO 90909
2. The original card was issued by another RTO that you are unable to contact but you have a copy of your original statement of attainment.
3. You have worked within the construction industry consecutively for the last 2 years.
